A negative culture should prompt more than repeated antibiotics
Persistent urinary symptoms do not prove bacterial infection; specimen quality, timing and noninfectious causes deserve review.
Several explanations may be worth exploring
- Bladder pain syndrome / ICBladder-associated pain with frequency can occur without positive routine cultures.
- Pelvic-floor myofascial dysfunctionMuscle spasm or tenderness can produce burning and urgency.
- Vaginal or urethral irritationLocal inflammation, atrophy or dermatologic conditions may mimic urinary infection.
- Sexually transmitted infection or urethritisSome organisms require tests other than standard urine culture.
- Sampling or antibiotic-related false negativePrior antibiotics or poor specimen timing can obscure bacterial growth.
